Complete your Admission

Once admitted to a graduate program within the College of Nursing, it is required that you send in the materials listed below.

Please send all requested materials to:

Send the following materials in to the UND CON:

1. Passport Quality Photo of Yourself


a. Upon admission to the Graduate Nursing program, all students must supply the Graduate Office with a digital photograph of themselves. The purpose of this photo is so we can clearly identify our students, especially those who are distance students.

b. Photos will be saved on the computer of the Graduate Nursing Student Service Specialist, as well as a copy of the photo will be printed and put in the front of students' academic file.

c. Photo may be used for online classes to be posted on Blackboard.

d. Specifications of the image include:

* .jpg format
* 8-bit color
* Range of width between 270 - 420 pixels
* 7 width by 8 height ratio
* Photo must be taken against a WHITE or LIGHT-COLORED SOLID background
* Photo must be in portrait format to include very top of shoulders and up
* Photo must NOT include hats or sunglasses or other objects that block the facial features
* Photos must be appropriate for the work setting
* File name should be specialization.last name.first name

(ex: FNP.Doe.Jane)

Use the following abbreviations for:

* Family Nurse Practitioner: FNP
* Gerontology: CNS-GERO or NP-GERO
* Psych-Mental Health: CNS-PMH or NP-PMH
* Public/Community Health: CNS-PCH
* Nurse Anesthesia: ANES
* Nurse Education: EDU
* PhD: PhD


e. Photos must be turned into the Graduate Nursing Office by the end of the first week of classes during students' first semester.

2. Immunizations Records

a. Hepatitis B Series AND Titre or Completed Waiver
b. TB Skin Test (within the last year)

i. This is to be required yearly
ii. If you have had a positive result in the past, please contact Pam Henderson for further direction.

3. Current RN or APRN License (from within the US)

a. Upon each renewal of your license a new copy must be sent to the College of Nursing at the address listed above

4. Insurance

a. The University of North Dakota College of Nursing provides liability insurance for all students -- on or off campus -- each semester they are attending the CON and are involved in official course work in clinical settings.

i. This includes those who are taking prescribed clinical nursing courses during the summer session. THIS DOES NOT cover vacation or other times when students are working for pay.

ii. Each graduate student is covered up to $1,000,000 for any one incident and up to $3,000,000 as a total of all claims within one calendar year period. (Source: CON Policies and Procedures Manual, #247)

iii. Graduate students may also choose to purchase their own additional professional liability policies.

5. Verification of Certifications

a. BLS (all programs)
b. PALS, ACLS (Anesthesia only)
